Interested applicants, please submit the following documents:

1. Application letter addressed to,
   CHERYL A. GOTINGA, MD, FPPS
   Medical Center Chief II
2. PDS and Work Experience Sheet downloadable from the CSC website;
3. Copy of Authenticated License or CSC Eligibility;
4. Copy of your trainings (if applicable);
5. Copy of your latest IPCR with at least a VS standing (if applicable);
6. Copy of your TOR and Diploma;

Please provide 2 sets of the following documents in a white folder and submit to the HRMO Training Office not later than October 5, 2023.

NOTE: INCOMPLETE DOCUMENTS WILL NOT BE ENTERTAINED AND WILL NOT BE CONSIDERED FOR THE NEXT HIRING PROCESS.
